When an item room would be generated with two full pedestal items but be given a third due to an effect like More Options or alternate floors, the "real" second item will ALWAYS be on the right, while the "choose one or the other" pedestals will always be on the left.
I haven't gotten four items enough to know what happens in that case (since that requires having More Options, going to alt floors, AND getting the rare double item room spawn all at once which has happened to me maybe twice in my like 400 hours of Repentance experience?) but I'm fairly confident it would follow a similar pattern.

Hey Matt, watching your videos inspired me to play Isaac again! I have full hardmode checkmarks as Isaac and I'm working on the Lost next, I've only got to do Mother, Delirium and Ultra greed. (and as a tribute I ALWAYS pick up lazy worm, even if I have a better trinket! Except perfection) 😭
If you’re talking about mod stuffs can you do a run with the Siivagunner sound pack and just not mention it, try to ignore the changes and see if anyone notices
